Rehabilitation / Senior Day Care Services

HWA provides integrated rehabilitation and senior care services designed to support individuals with functional limitations, ageing-related needs, and long-term conditions. Our programmes are delivered by trained professionals in a safe, structured, and person-centered environment.

Day Rehabilitation

Our Day Rehabilitation Centres serve adults and older adults with physical disabilities, helping clients regain strength, mobility, and confidence for daily living. Services under our Day Rehab Programme include:

Physiotherapy (PT)

Our licensed physiotherapists assess and treat movement problems caused by disability, trauma, or illness. This includes guided exercises and therapies to improve mobility, balance, and strength. Sessions are tailored to each client’s needs to restore physical function and reduce pain.

Occupational Therapy (OT)

Our licensed occupational therapists help clients relearn or adapt the skills needed for daily activities and work. This may involve therapy to improve fine motor skills, training in self-care tasks, and practical modifications to the home or environment. The goal is to enable clients to maintain or regain independence in daily living, ensuring they can perform activities such as dressing, eating, or vocational tasks safely and confidently.

Community Rehabilitation (Active Rehab)

This intensive, goal-oriented program supports clients requiring active therapy post-hospital discharge or during recovery. It integrates physiotherapy and occupational therapy to enhance skills such as mobility and coordination. Clients participate in regular weekly sessions following a tailored plan to promote community reintegration. Our multidisciplinary team works closely with each individual to achieve personalized rehabilitation goals.

Maintenance Exercise

Clients who have completed active rehabilitation can continue with our Maintenance Exercise Programme. It helps preserve physical function and prevent decline after initial rehabilitation. Sessions are less intensive but held regularly to promote long-term wellness. Engaging in supervised activity helps clients maintain independence and optimal health.

Senior Day Care Services

HWA’s Senior Day Care Centre offers a safe, structured daytime program for older adults, including those with dementia. Our goal is to help seniors stay engaged physically and mentally while providing caregivers with peace of mind throughout the day.

Maintenance Day Care (MDC)

MDC is an all-day program for seniors featuring engaging activities, gentle exercises, and personal care in a social setting. The care team promotes activity and connection through events like light workouts, arts and crafts, music, games, and outings. They assist with meals, toileting, and medication reminders. Designed for frail or socially isolated seniors not needing full nursing care, it offers supervised activities and companionship, fostering physical, mental, and social well-being in a welcoming, home-like environment.

Dementia Day Care (DDC)

Specialized service for seniors living with dementia. We offer engaging activities, memory exercises, and routines designed to reduce confusion and anxiety. The center is dementia-friendly with a secure, structured environment. Clients participate in reminiscence therapy, puzzles, music therapy, and sensory activities under the supervision of trained staff. Personal care, including meals and hygiene, is provided with dignity.

Severe Disability Assessment

Care Shield Life

HWA is an MOH/AIC-accredited provider of severe disability assessments to apply for long-term care severe disability financial assistance schemes administered by the Agency for Integrated Care (AIC) such as:

  • Care Shield Life
  • Elder Shield
  • Elder Fund
  • Interim Disability Assistance Programme for the Elderly (IDAPE)
  • MediSave Care

Centre-Based Assessment (S$100)

Available at our professionally equipped centers in Whampoa and Jurong Point.

Home-Based Assessment (S$250)

For clients who are bedbound or have difficulty travelling, our MOH-accredited Severe Disability Assessors can conduct the evaluation in the comfort of your home.

To arrange a Severe Disability Assessment with HWA, clients or caregivers can contact our centers to book an appointment. We will advise on the required documents and the assessment fee. Do take note that we do not have a doctor in our center to assess Mental Capacity Assessment.

Service Fees: Day Rehab, Senior Day Care, Transport

HWA’s rehabilitation centres are accredited by the Ministry of Health (MOH) as step-down care providers for subsidised community rehabilitation services and Senior Day Care Services. Rehabilitation, Senior Day Care and transport fees are determined by the MOH Household Means Testing (HHMT) outcome, which assesses each client’s subsidy eligibility.

Day Rehabilitation Services

  • First-Time Assessment

  • Rehabilitation Session

(Physiotherapy / Occupational Therapy)

: Fees range from S$16.80 to S$196.20 (inclusive of GST) per session

  • Maintenance Exercise Sessions

(Physiotherapy / Occupational Therapy)

: Fees range from S$19.00 to S$81.75 (inclusive of GST) per session

Transport Services

For clients using HWA’s wheelchair-accessible transport, charges are also subsidised based on HHMT eligibility.

from S$5.60 (highest subsidy) to S$54.50 (full fare), subject to availability.

Senior Day Care Services

: Daily fees range from S$10.80 to S$86.11 (inclusive of GST).

  • Dementia Day Care

(Full Day)

: Daily fees range from S$12.40 to S$105.73 (inclusive of GST)

Fees are based on attendance of 5 days per week (Monday to Friday). Fees will be collected on a monthly basis for all service type.

Subsidies

HWA is part of the national network of care providers under the 

MOH /AIC.

  • Singapore Citizens: Eligible for up to 80% subsidy (lowest income tier)
  • Permanent Residents: Eligible for up to 55% subsidy

MediSave Usage

Clients may use MediSave for eligibleCommunity Rehabilitation (Active Rehab) Services, in accordance with MOH guidelines.

  • Up to S$25 per day

  • Capped at S$1,500 per year

Deposit Policy

For day rehabilitation and day care services, a refundable deposit equivalent to one month’s fees is collected upon commencement.

  • The deposit will be used to offset the final month’s fees upon service cessation

  • No GST is charged on the deposit

  • A one-month advance notice is required for withdrawal to allow refund processing
